Abstract

Chronic diseases, including cardiovascular diseases, diabetes, cancer, and respiratory disorders, are among the leading causes of morbidity and mortality worldwide. Modern clinical practice requires a comprehensive and integrated approach to managing these diseases through early diagnosis, personalized treatment, and long-term care. This paper explores the significance of combining predictive diagnostics, biomarker analysis, genetic profiling, and digital health tools to enable early intervention and improve patient outcomes. The literature review synthesizes pre-2019 evidence on innovative diagnostic techniques, targeted therapies, and management strategies. Key findings emphasize the role of integrated care models, multidisciplinary teams, and technology-driven solutions in enhancing chronic disease outcomes. Future directions include expanding precision medicine approaches and improving patient engagement through artificial intelligence and machine learning.
